While you are more than welcome to register with one of our brocheres at the center, if you would like to register online instead
simply fill out the registration form below and hit the "Submit" key.  Upon completion of your registration, you are also welcome
to pay at the front desk of your child care center or use the "Pay Online" link above on our website if you would like.  (Please
note that registered dancers will not be allowed to attend class until the initial payment is received.

Please read the following, initial in the space provided and sign the consent
form below.
By its very nature the activity of dance carries a risk of physical injury.  I
understand that no matter how careful the instructors are that inherent
danger cannot be eliminated.
All tuition is due the last day of each month for the following month.  I
understand that payments made after the 5th of each month will be assessed
a $10.00 late fee, and that students with an unpaid balance may be removed
from class and their spot could be taken.  I also understand that Dance
Dreams is a voluntary activity, and I agree to pay for my child's tuition
regardless of absenses, and I will be responsible for my child's tuition until a
written letter is provided to Dance Dreams stating that my child is no longer
attending.  There are no makeup classes for student absenses.  A $25.00
fee is charged for all returned checks.
I understand that occasionally photos or videos of various classes or
students will be used for marketing and display purposes.  (Note: Student's
names will not be given without written permission for parent or legal
guardian.)
